0fa96df818 Remove DriveState::DiscChangeDetected hack
Since both GameCube and Wii resets now work correctly, this hack is not needed anymore.

b8715b42d2 Only reset drive chip state (not DI registers) when changing discs
Resetting the DI registers disables interrupts, which means any errors reported (for instance) are just not sent though.

537e40afb5 Only update DIMAR and DILENGTH if transfer completed without error
Turns out, Gamecube games actually do check DILENGTH, and if DILENGTH is at 0, they'll think the transfer completed successfully even if DEINT is used, since after all, surely that means everything was sent.  That caused all sorts of issues, from audio looping when a disc is removed since it's re-using the same buffer to just flat-out crashing instead of showing the disc removed screen.

fba51b2956 Split drive state and drive error into 2 fields, and fix some inaccuracies
In particular:
- Trying to play audio in a non-ready state returns the state-specific error, not an audio buf error
- Audio status cannot be requested in non-ready states
- The audio buffer cannot be configured in states other than ReadyNoReadsMade
- Using the stop motor command while the motor is already stopped doesn't change states

Additionally, the internal state IDs are used (which distinguish ReadyNoReadsMade and Ready), instead of the state IDs exposed in request error.  This makes some of the weird behavior a bit more obvious.

State and error behavior of the seek command was not implemented in this commit.

162e3be82b Show an OSD message when running a disc image with a large block size
This is intended to catch WIA files which have been created using
wit's default parameters (40 MiB block size), once the WIA PR is
merged. The check does however also work for GCZ files – not that
I think anyone has a GCZ file with a block size that large.

94f83db2b5 Adjust s_DIMAR/s_DILENGTH behavior (fixes Baten Kaitos music)

The problem seemed to be that s_DILENGTH would get set to 0
at times when it shouldn't. Simply not changing it in case
of NoReply or DTK seems to fix the problem. However, we can
actually go one step further in accuracy and use data.size()
to change s_DIMAR and s_DILENGTH as partial reads (NoReply
commands) complete, instead of jumping directly to 0 when
the whole read completes.

21c152f51f Fix Error #001 (alternative solution)
This is an alternative to PR 8557 and PR 8558. The way this PR solves
the problem is essentially the same as what we had before PR 8394
(except the code we had back then only worked because it was broken).

6c0399103f Handle partitions in /dev/di, not DVDInterface
Partitions are Wii-exclusive, and don't happen at the DVDInterface level in
IOS.  This isn't quite the cleanest fix, but it gets rid of the assumption that
a partition is open on starting the game at least.

7d6b9bcb40 Check for error 001 out of bounds reads in DVDThread
All out of bounds reads should return the appropriate DI error, but it also
makes sense to have the error 001 read happen there.

ef2fc5a49b Split /dev/di commands from DVDInterface
The various ioctls sometimes have different arguments than the DI command
registers, though they generally overlap.  There are also a bunch of ioctls
that don't even normally go into DVDInterface, just returning various data.
Some of the implemented ioctls are new to Dolphin.

d3aad1d6d5 DIMAR only ignores bits 0-4, not the upper bits
Based on a hardware test on a Wii.  The alignment code was originally added in 743641965a2.

c564d64104 Remove unused drive debug command constants
The corresponding code that used these was removed in 2009 with 93b83f8d659.  Now their only purpose is to generate warnings on osx.

Minimalistic documentation on these commands can be found at http://hitmen.c02.at/files/yagcd/yagcd/chap5.html#sec5.7.2 and https://web.archive.org/web/20070328200323/http://tmb.elitedvb.net/dvd-game/index.php/CMDFE.  Those constants only relate to the 0x11 subcommand, which is one of many.  Most can't be properly emulated unless we LLE the drive firmware (in which case, they don't need to be reimplemented).

c792961000 Common: Unify logging namespace with Common
Previously the logging was a in a little bit of a disarray. Some things
were in namespaces, and other things were not.

Given this code will feature a bit of restructuring during the
transition over to fmt, this is a good time to unify it under a single
namespace and also remove functions and types from the global namespace.

Now, all functions and types are under the Common::Log namespace. The
only outliers being, of course, the preprocessor macros.

3efa3d7a42 Fix 2 DTK bugs
Bug 1: subcommand 1 of 0xE1 does not actually reset the current position; it just stops playback.  0xE2 returns the same value it would have before stopping playback (other than reporting that the stream is stopped).

Bug 2: subcommand 3 of 0xE2 was incorrectly dividing the length by 4; while this makes sense for positions (which are multiplied by 4 earlier to get a byte offset), it is not correct for lengths.

0f5a4b37ee DiscIO: Add functions CreateDisc and CreateWAD
...in addition to the existing function CreateVolume
(renamed from CreateVolumeFromFilename).

Lets code easily add constraints such as not letting the user
select a WAD file when using the disc changing functionality.

3ab74f8cb7 HW/DI: Emulate rotational latency
This aims to emulate rotational latency as part of the disc drive
timings, which makes loading times have more variance (like on a
real disc drive) but should not affect the average loading times.

352ac91a1c Add a delay before automatically switching discs
Some games don't behave as expected if we eject the disc as soon as
we receive the DVDLowStopMotor command. For instance, Baten Kaitos
never shows the prompt to switch discs or the "Reading disc..." text
(but works correctly other than that).

20b8c24847 Fix first DTK track not playing after booting with IPL
DVDLowAudioBufferConfig doesn't start a DTK stream, it just tells the
drive that the game is allowed to use DTK. At least if I'm not mistaken.

58743416bb Make the support for unencrypted Wii disc images less broken
These disc images are only used on dev units and not retail units.
There are two important differences compared to normal Wii disc images:

- The data starts 0x8000 bytes into each partition instead of 0x20000
- The data of a partition is stored unencrypted and contains no hashes

Our old implementation was just guesswork and doesn't work at all.
According to testing by GerbilSoft, this commit's implementation
is able to read and extract files in the filesystem correctly,
but the tested game still isn't able to boot. (It's thanks to their
info about unencrypted disc images that I was able to make this commit.)

1b3dae918a StreamADPCM: Turn the ADPCM decoder into a class
Migrates the state to be instance-based as opposed to being a flat
namespace. This keeps behavior localized to its own instantiable unit
(and forces uses of the class to also be localized, lest they cart around
an instance all over the place).

0e2690f40b FileMonitor: Store last file offset instead of path
This saves us from having to call GetPath when the same file is being
read over and over. (GetPath is more expensive than GetOffset due to
it iterating through parts of the file system and creating strings.)

b00ef39c1c Fix DTK audio not working after loading a savestate
The main problem was that the volume of the mixer wasn't savestated.
The volume is typically 0 at the beginning of a game, so loading a
savestate at the beginning of a game would lead to silent DTK audio.

I also added savestating to StreamADPCM.cpp.
